HOW TO ATTEND

  • Fax your registration 24 hours/day to (505) 272-8604.
  • Call (505) 272-3942 between 8:00 am and 5:00 pm M.S.T.
  • Mail your registration to: University of New Mexico
    Office of Continuing Medical Education.
Participants requiring special accommodations should contact the Offi ce of Continuing Medical Education at (505) 272-3942, as early as possible. UNM is in compliance with the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990.


REGISTRATION INFORMATION

Your registration fee includes conference materials, the reception, continental breakfast and the luncheon. Minimum and maximum numbers have been established for this conference; therefore, your pre-registration will be confi rmed by mail. Please confirm your registration before making travel arrangements. Present your confirmation letter at the registration desk. It is UNM policy to charge the off erer $15.00 plus normal merchant bank fees for each returned check.

CANCELLATION

If you pre-register and cannot attend, the Office of Continuing Medical Education will refund tuition, less a $40.00 administrative fee, provided it is received in writing on or before October 3, 2008. No refunds will be issued after this date or for non-attendance.

Conferences are subject to cancellation. In the event this conference is cancelled, the Office of CME is not responsible for any airfare, hotel or other costs incurred by participants.
